Who we are:
With over 20 years of experience providing rehabilitation services in post-acute care, our team identified a need for a comprehensive therapy program for patients in senior living communities. Our mission to deliver continuity of services and support CMS' value-driven model allows us to implement a full-circle approach to therapy. We work with the appropriate partners in assisted living communities, independent living communities and home health agencies to execute a comprehensive medical model that meets the needs of patients specific to their setting.

What we do:
Element provides functional, evidence-based rehab services to individuals living in AL, IL and their home. Our clinical services are relevant to the environment where our patients live, and goals are established in collaboration with the patients and families to ensure all concerns are addressed.
